What Is It?

Continuing or new pain after spine surgery due to scar tissue, adjacent-segment disease, hardware issues or unrealized diagnoses.

Symptoms

  • Axial or radicular pain > 6 months post-surgery
  • Neuropathic features
  • Functional decline

Our work-up

  • High-resolution MRI with metal artifact reduction
  • Diagnostic blocks
  • CT myelography when indicated

Treatment at PVI

  • Adhesiolysis (epidurolysis) with targeted steroid delivery
  • Spinal cord or dorsal root ganglion stimulation (SCS/DRG)
  • Intrathecal pain pump for mixed nociceptive/neuropathic pain
  • Multidisciplinary pain psychology & rehab
    Goal: regain quality of life without repeat open surgery.
